Pay FGA Fee

This is the fee to have your case reviewed by Dr. Semelka to see if you qualify for the Functional Genomic Analysis study with Dr. Bob Miller. Please follow any additional instructions listed in the blog post regarding this study after paying this fee.
 
*Please note that if you have already paid the $300.00 consultation fee and consulted with Dr. Semelka in the past, you do not need to pay this additional fee.
 
*Instead of paying this fee, you may instead sign up for an annual membership with GadTTRAC for $50.00 at www.gadttrac.org. This will allow you entry into this study, and any other opportunity that arises within the year from Dr. Semelka or GadTTRAC.
